penumbra® has issued an urgent recall on its jet 7 xtra flex® catheter and the jet 7max series®, both of which are used on those that are suffering from an emergency ischemic stroke. over 200 cases of adverse side effects including injury and death have been reported to the fda. the Devices failure report lists "Ballooning, expansion, rupture, breakage or complete separation, and exposure of internal support coils near the distal tip region of the JET 7 Xtra Flex® catheter", according to the FDA's official notice.

Estimates provided by penumbra® state that 30,000 defective units have been distributed in hospitals across the country. This is not the first time penumbra® has come under fire for a dangerous medical product, as the company released a 3d stent retriever that may break during use in 2017.
